Important read carefully before submitting form. The artwork and specifications we sent you is for your approval. Your order will be custom made from the original of this art and proposed specifications. If you approve the information as correct, you will be financially responsible for the order. If you do not understand the information or terms used, do not submit as approved. Submit your questions for answers first on this form. Make sure you get any questions resolved before the order is approved. We have tried our best to meet all of your expectations but there is always the possibility that we could have made a mistake. You are a very important client to us. It is very imperative that you examine this art ( locate your order # on this page to view final proof) and order confirmation (in email sent you)specifications carefully before you approve it. Please check for accuracy in all of the following areas;

WORDING - Did we say what you wanted to say? Is the punctuation as you would like it? Did we use capitals or italics as wanted? etc.
SPELLING - Is everything spelled correctly? Check all names of towns & organizations etc.
LAYOUT -Drawing is not actual size. All components will relate to each other as they do in your proof. If you are unhappy with spacing, letter size, line breaks or any design aspect, THIS IS THE TIME to change it!
SIZE - Sizes of your order were sent to you in your order confirmation. If this is not what you ordered LET US KNOW NOW!
MISC. - Does it look like you think it should? Is there anything else you have noticed that doesn't seem right?
Significant changes in the original order specifications provided by client will/may result in additional charges to cover the cost of rework. Rework from the original specifications will/may result in additional production time and will/may delay he planned delivery.
